To be eligible for medical cannabis treatment, we will need to verify that you qualify for the CUP program through the State of Texas. Please fill out the following screening questions as accurate as you can so that we can verify and/or qualify you for eligibility.

If you have medical records that you can share with the physician that will qualify you for the CUP program you may attach them at the end of the screening.

If you are currently being seen by a physician or psychiatrist please list their name and telephone number so that we can provide the best continuity of care.
